A covert failure is an unannounced failure which is neither obvious to operators nor detected by means of diagnostics. Covert failures are only revealed by subsequent events, such as failure to meet a demand or by testing of the system.
As the process industries experience the “great shift change,” developing the next generation of leaders requires more than technical competence.
